Requesting a Leave of Absence

If you need to take an extended leave, please download the Leave Request Form and then email it to cmphr@cacmp.org. Our HR team will then reach out to you to schedule a meeting to discuss your leave request and walk you through the process.

If you have any change to your personal data, please send an email to cmphr@cacmp.org so that we can send you the proper forms to fill out.
